It is a comic strip after all. And they're usually printed on paper. He could have been implying dilbert should have cut the question out so he wouldn't have to answer it. Although, I think it may be funnier wondering what the boss means rather than knowing.

My first impression was the PHB was expecting Dilbert to cut his own tongue rather than answer the customer. I don't think I have become wiser even after going through all the comments though a couple of them also thought similarly.

I was thinking he simply meant Dilbert could have poked himself as a distraction, though knowing PHB, he meant Dilbert could've cut his tongue out so he couldn't answer her.
